Try google:

I think the first link has info on where to sign up, but it's down right now.
Level 4? It's pretty simple. If you've completed a year of University Level of Japanese, you should do okay.

I have a pamphlet for the JLPT right here;
for the level 4 test:
"The examinee has mastered the basic elements of grammar; knows around 100 kanji and 800 words; has the ability to engage in simple conversation; and to read and write short, simple sentences. This level is normally reached after studying Japanese for approximately 150 hours and after completion of the first half of an elementary course.
60% (240/400 points) is a passing score.
http://www.jflalc.org/?act=tpt&id=8 is the home page for the test. It is given in Columbia University in New York. The test will be given this year on December 4th, last day for registration is October 7th. Level 4 would cost you $40.
