After trying to get info here and there i'm pretty lost and confuse.

1) I only have 6 day 5 nights in Tokyo for spring. Not sure if i shld/can add another city into my schedule. Already got my air tkt and 3 nites stay in Tokyo done. Now not sure to stay in Tokyo or out of Tokyo on my last 2 nites

2) Am totally confuse with the type of transport passes available in Japan. For my situation shld i get a "Suica & N'EX Card" in Japan or get a "JR Pass" in my country. Base on wat i get from (jreast website) "Suica & N'EX Card" is more for the use in Tokyo city am i rite?

3) I tot of planning my schedule as followed

Day 1 - Full day either at Disneyland or Sanrio Puroland (Have been to Disneyland in LA is it about the same? Which one is more recommended?)
Day 2 - Full day tour to Mt.Fuji, Hakone & Return by Bullet Train offer by(hisexperience.jp)
Day 3 - Shopping in Shibuya and Harujuku (Any other place to shop for clothing but not too expensive? Is there any wholesales market in Tokyo?)
Day 4 - Tot of checking out the onsen and/or ryokan (Any recommendation? There are so many onsen but not too many ryokan in Tokyo and i tot Hakone may be a better choice shld i jus strike out my Day 2 tour?)
Day 5 - Visit the Shrine (Is Meiji Shrine the most popular one in Tokyo?) and any other places where i shld visit?
Day 6 - My last day in Japan till i take off @ 1900hrs.

My head is getting burst and i only have a week + to plan where to go and how much to budget for those extra expenses cos i'm pretty grey with my Day 4 schedule onwards not sure if i have spend too many days in Tokyo.

Hopefully to receive some precious recommendation and/or other options. Thanks

If you're only going to be in Tokyo, forget the JR Rail Pass.

I'd suggest spending at least ONE day outside Tokyo, perhaps two if you can swing it. Different strokes for different folks, though. (Seems a total waste of a trip to spend any time at all at Disneyland, especially if you have less than a week to spend in country.)

Should you plan one more day to your itinerary? That's all up to you. You could spend months here as a tourist and still want for more!

The one-day trip to Hakone and Mt. Fuju will be cheap enough for you to skip the Rail Pass since you are spending the majority of your time in Tokyo.

Also, why go to Disneyland there since you've been to the one in the U.S.? From what I've heard, it isn't all that much different except Main Street has a roof.

I would go to Disney Sea instead, if you have to have an amusement park experience, or take a day-trip down to Yokohama.
